Fellowship Hall Rental

Our Fellowship Hall is available for special events.  Below is the forms for you to fill out to rent the Fellowship Hall.

 

Fellowship Hall Use Form

if you would like to reserve the hall please contact the church office at 419-665-2262 and confirm the date you would like to use the hall. All requests can be emailed to the church here.

After you have confirmed your date below is the form you will need to fill out for the church records.

• To email your forms please click here.  This is a word 2010 document.  Please fill out the form then save as a PDF and email to the church office.
• To mail your forms to the church, please click here.  This is a PDF document.  Print form and then mail back to the church.
